What we collect and why

We collect your name, mobile number, optional email, preferred language and voucher details to activate your voucher, prevent repeat use, arrange your appointment and record redemption. We also record when you accepted this notice. Please do not enter medical history, identity-document numbers or payment information.

Who can access your details

Access is limited to authorised Dr Ultrasound Centre staff and the technology providers that operate this registration service. Your registration details and medical information are not shared with your insurance agent or the event’s insurance partner through this system. The partner distributes the voucher; Dr Ultrasound Centre provides the healthcare service. You do not need to buy insurance.

Contact and your choices

By checking the consent box, you agree that our team may contact you about your voucher and appointment using the details you provide. This does not give consent to unrelated marketing. Email is optional. You can contact the clinic to ask about access, correction, withdrawal of contact consent or deletion of your registration details. Withdrawal may prevent us from arranging your voucher appointment.

Storage and protection

The service uses managed hosting and database providers. Patient contact details are encrypted in the application database, and access to clinic records requires an approved staff account. Information may be processed outside Malaysia by service providers. We retain information for campaign administration and applicable record-keeping needs; contact the clinic for details of its retention policy. Security controls also limit repeated activation attempts.

Your voucher

The offer is RM30 off bilateral breast ultrasound. Activation is not a confirmed appointment. The expiry shown on your confirmation applies, and each voucher may be redeemed once. Our clinic team will confirm appointment availability and clinical suitability.
